Once a referral is received (either directly from you or your Home Care Package or Support at Home provider), here’s what happens next:
We get in touch to arrange a convenient time for your home visit.
We complete a comprehensive home assessment, looking at your environment and how you go about daily tasks.
We speak with you about what matters most—including self-care, household tasks, community participation, productivity, and leisure activities.
We identify areas for greater engagement, comfort, or independence based on your personal goals and lifestyle.
We provide a detailed report with tailored recommendations—this might include equipment, assistive technology, home modifications,
task retraining or education.We support equipment trials to ensure the recommended solutions are the right fit. Trials can take place at home, in the community, or with suppliers.
We work closely with your case manager and provider, ensuring everyone is informed and involved for the best possible outcomes.
We prioritise clear communication and a fast turnaround, so you're never left waiting.
