Once a referral is received (either directly from you or your Home Care Package or Support at Home provider), here’s what happens next:

  • We get in touch to arrange a convenient time for your home visit.

  • We complete a comprehensive home assessment, looking at your environment and how you go about daily tasks.

  • We speak with you about what matters most—including self-care, household tasks, community participation, productivity, and leisure activities.

  • We identify areas for greater engagement, comfort, or independence based on your personal goals and lifestyle.

  • We provide a detailed report with tailored recommendations—this might include equipment, assistive technology, home modifications,
    task retraining or education.

  • We support equipment trials to ensure the recommended solutions are the right fit. Trials can take place at home, in the community, or with suppliers.

  • We work closely with your case manager and provider, ensuring everyone is informed and involved for the best possible outcomes.

  • We prioritise clear communication and a fast turnaround, so you're never left waiting.
