Support at Home Funding Classifications

Under the new program, participants will be assessed and allocated to one of 8 ongoing funding classifications, ensuring support is tailored to individual needs. This system improves upon the previous four-tier HCP model by offering more nuanced levels of support.

Participants can carry over unspent funds up to $1,000 or 10% of their quarterly budget, whichever is greater. For more information, click this link:

The 8 Ongoing Funding Classifications:

  1. Classification 1: For individuals requiring minimal support to maintain independence.

  2. Classification 2: For those needing basic assistance with daily activities.

  3. Classification 3: For individuals requiring moderate support, including some allied health services.

  4. Classification 4: For those needing regular assistance and multiple services.

  5. Classification 5: For individuals with higher support needs, including complex care coordination.

  6. Classification 6: For those requiring substantial assistance with most daily activities.

  7. Classification 7: For individuals with significant care needs, including frequent clinical interventions.

  8. Classification 8: For those with very high care needs, exceeding the current Level 4 HCP, with an indicative annual budget of up to $78,000 .

Note: The exact services and funding amounts for each classification will be determined based on individual assessments conducted through the new Integrated Assessment Tool.

Short-Term Care Pathways

In addition to the ongoing classifications, the Support at Home programme includes 3 short-term care pathways designed to address specific, time-limited needs:

  • Restorative Care Pathway: Focused on helping individuals regain function and independence after a health setback.

  • End-of-Life Care Pathway: Providing palliative care services to support individuals with life-limiting conditions.

  • Assistive Technology and Home Modifications (AT-HM) Scheme: Offering equipment and home modifications to enhance safety and mobility, with funding tiers based on assessed need. The AT-HM scheme provides funding for essential equipment and home modifications to support daily living activities. This includes items like mobility aids, bathroom modifications, and other assistive technologies. Health.gov.au

    • Funding Tiers:

      • Low: Under $500

      • Medium: Up to $2,000

      • High: Up to $15,000

  1. Contact My Aged Care: Begin by reaching out to My Aged Care at www.myagedcare.gov.au or call 1800 200 422.

  2. Assessment: Undergo an assessment to determine eligibility and appropriate funding classification.

  3. Service Planning: Collaborate with your chosen provider to develop a tailored care plan that meets your individual needs and goals.

Once a referral is received (either directly from you or your Home Care Package or Support at Home provider), here’s what happens next:

  • We get in touch to arrange a convenient time for your home visit.

  • We complete a comprehensive home assessment, looking at your environment and how you go about daily tasks.

  • We speak with you about what matters most—including self-care, household tasks, community participation, productivity, and leisure activities.

  • We identify areas for greater engagement, comfort, or independence based on your personal goals and lifestyle.

  • We provide a detailed report with tailored recommendations—this might include equipment, assistive technology, home modifications,
    task retraining or education.

  • We support equipment trials to ensure the recommended solutions are the right fit. Trials can take place at home, in the community, or with suppliers.

  • We work closely with your case manager and provider, ensuring everyone is informed and involved for the best possible outcomes.

  • We prioritise clear communication and a fast turnaround, so you're never left waiting.
